Contact our office to make an appointment at 972-420-1475. Our staff will ask for patient information, insurance company, and appointment detail preferences. They will invite you to our patient portal, My Kid’s Chart, where you can request appointments in the future, update your insurance card, change your demographics, send us a message, and more!
