What is CUB Advantage by MD Pediatric Associates?
Direct Primary Care is a model of health care that fosters the physician-patient relationship, emphasizing personalized care without the constraints of health insurance. One of the important benefits of the pediatric direct primary care model is that it allows us to provide comprehensive pediatric care with a full range of personalized services through a low monthly membership.
What is included in the CUB Advantage membership?
Same Day Appointments
Telehealth Visits (one well visit in office is required per year)
24/7 Medical Advice
Well Child Examinations
Routine Immunizations including annual influenza vaccination
Portal Account that offers access to telehealth, immunization records, comprehensive problem and medication lists, the ability to request appointments online (virtual or in- office), the ability to securely ask questions regarding your child’s health and to receive direct communication securely from the MDPA team.
Routine vision/hearing screening
Infant/Toddler Fluoride Application at well exams
Sports Physicals and Form completion
What is not included in the CUB Advantage membership?
Urgent Care/Emergency Room visits
Pharmacy serviced Medications
Non-routine immunizations and injections
Procedures (circumcision, ear piercing, splints, etc)
Specialist Visits (Including but not limited to: OT, PT, ST referrals, EEG, MRI/CT/Xray, Counseling referrals, Orthotic Needs, Specialist referrals, etc)

I have health insurance, how does this membership benefit my family? Why would I pay ‘extra’ for a membership with MD Pediatric Associates when I already have health insurance?
Health insurance is not health care. We believe that health insurance policies can put barriers in the way of quality, individualized care. We created CUB Advantage with the goal to transform the traditional doctor’s visit to a more personalized, convenient health care experience for your family. We want to spend time with our patients and get to know them, so we can provide excellent care throughout their growth and development in each child’s life.
Do I need insurance in addition to this membership? Can I drop my current policy or health sharing plan?
We highly recommend that you have insurance coverage. While we provide extensive pediatric services, there can be occasions that a child will need healthcare outside of our facility or medications and procedures at our facility which are not part of the CUB membership.
We just don’t go to the doctor very often. Will a membership to Cub Advantage be worth it?
Childhood illnesses and injuries are unpredictable. Just as you may have previously paid a monthly fee for insurance “just in case”, we consider this type of care worth your investment. We anticipate this membership will help eliminate surprise expenses for your children’s medical care.
Can I try the membership before I decide to pay for it?
We do not offer trial periods for this membership, but Kimberly Kinkaid, our Practice Manager, is available for any and all questions you may have before enrolling into the membership. Please contact her at 972-420-1475 ext. 1209 or via email at email@example.com.
Can I see the specialists of my choice?
Specialists visits are not included in the CUB Advantage membership, therefore you have the right to choose whichever specialist you want to see. You may be limited to a network depending on whether or not you have other insurance restrictions.
Can we see any clinician we want with the membership?
Yes, all clinicians at MD Pediatric Associates accept the Cub Advantage membership.
Is this concierge medicine?
Although this may seem a lot like concierge medicine, the CUB Advantage membership does not have the higher costs generally associated with concierge medicine. A direct primary care model differs in that concierge medicine usually involves a relationship with a single provider, but the direct patient care model allows you to form relationships with all of our providers and staff, enabling you to receive more timely care.
Do you accept Medicaid?
We do not currently accept Medicaid, CHIP, or any government assisted insurance plans.

How do I register online as a new patient?
Click the appointment button and click the ‘New Patient’ Button. The forms will come to our office and we will contact you to schedule.
How do I obtain a Follow My Health (FMH) portal account?
Call the office (972.420.1475) and speak to the schedulers or IT to get an invitation for your child(ren).
How do I ask a medical question to my provider, request ADHD meds, or view my records?
Send a message to us with the Follow My Health portal, starting either from our
website or with the app on your smartphone.
How do I get a copy of my shot record?
You can print it off of your Follow My Health portal account, or request a signed copy
by sending a Follow My Health portal message.
